Month 1–6
Arrival & Stabilization
A Warrior moves in. 24-hour trauma-informed care, an informed nurse on call, and consistent transportation to hematology appointments begin to reduce ER reliance and pain-crisis frequency.
Year 1
Nutrition & Routine
Farm-to-table meals, sleep hygiene, and a predictable rhythm lower inflammation and stress. Education re-engagement and weekly financial-literacy workshops begin.
Years 2–3
Education & Identity
Career and health speakers, arts, language, and mentorship build confidence. Custodial savings account compounds. First study-abroad or enrichment milestone.
Years 4–5
Skills & Independence
Internships, vocational placement, and life-skills mastery prepare the Warrior for life after the home — with a personalized aging-out plan in motion.
Years 6–10
Transition & Community
Long-term housing referrals, continued health navigation through age 27, and an alumni network. Many Warriors return as mentors, partners, and advocates.
